Parental Consent Form
For junior members under the age of 16.

Why we need this form
Under Section 31 of the Data Protection Act 2018, the age of digital consent in Ireland is 16. Where Tenpin Ireland relies on consent to process personal data about a child under 16, that consent must be given by a person with parental responsibility for the child.
Even where we rely on a different lawful basis (for example, to administer your child’s membership), we ask you to confirm that you understand what we do with your child’s personal data, and to give specific consent for any activity that goes beyond the basics — for example photography or marketing communications.

Information collected on the form
The form records the following details:
- About your child: full name, date of birth, club, and membership category (if known).
- About you (parent or guardian): full name, relationship to the child, address, email and telephone.
Your consent
On the form, parents and guardians indicate YES or NO to each of the following:

| B | Tenpin Ireland may share my child’s personal data with affiliated bowling centres, with Sport Ireland, Sport NI, and with international federations (International Bowling Federation and the European Bowling Federation) where this is necessary to administer the sport. |
| C | Tenpin Ireland may publish my child’s competition results on the website on a current-and-previous-season basis, in line with the Children’s Data Policy. |
| D | Tenpin Ireland may take and use photographs and/or video of my child at events for the legitimate purposes of the sport, including reasonable promotion of tenpin bowling, in line with the Children’s Data Policy. |
| E | Tenpin Ireland may send operational communications (membership renewals, event information, team announcements) to me as the parent/guardian by email and/or post. |
| F | Tenpin Ireland may send me an occasional newsletter and information about Tenpin Ireland’s wider activities by email. (Optional.) |
Health and special category information
If your child has a medical condition, allergy, or any other matter that we need to be aware of in order to support their participation safely, the form provides space to describe it. This information is processed under Article 9(2)(a) GDPR (explicit consent) and is shared only with those who need to know. The form includes a specific tick-box giving explicit consent to process this health information for the purposes described.
Your rights
You and your child have a number of rights under the GDPR and the Data Protection Act 2018, including the right to access, correct, or delete your child’s personal data, and the right to object to or restrict processing. You may withdraw any consent given on this form at any time, without affecting the lawfulness of processing carried out beforehand. Verification
Tenpin Ireland may take reasonable steps to verify that the person signing the form is in fact a parent or legal guardian of the child named. We do this to protect your child.
Declaration
By signing the form, the parent or guardian confirms that:
- They are the parent or legal guardian of the child named.
- They have read the Privacy Policy and the Children’s Data Policy.
- The information given on the form is accurate to the best of their knowledge.
- The consents given reflect their decisions, freely made.
Child acknowledgement (where age-appropriate)
The form includes an optional space for the child to acknowledge that their parent or guardian has talked to them about it, in a way they understood, and that they are happy for it to be signed on their behalf. This is your data and you have rights about how it is used — you don’t have to sign if you don’t want to.
